When no psychotic symptoms are present my approach to the treatment of people with BPD can be summarized as:

(carbamazepine and/or valproate) + (high dose SSRIs or MAOIs).

One of the first symptoms to improve is anxiety.

Pharmacotherapy of patients with borderline personality disorder is based on the identification of target symptoms. The usual target symtpoms that have been shown to respond to medications are impulsivity (usually responds to carbamazepine), affective instability (usually treated with lithium, MAOIs or SSRIs), cognitive disruption (criterion 9 in DSM-IV, usually treated with low dose neuroleptics), and more recently SSRIs have been shown to be effective in decreasing the anger outbursts of patients with BPD (J Clin Psychopharmacol 15 (1), Feb 1995, 23-29). The best study on the pharmacotherapy of borderline personality disorder remains the 1988 paper by Cowdry & Gardner (Arch Gen Psych 45: 111-119).

I have used [gabapentin] in patients with borderline personality disorder + bipolar disorder, often combining it with high dose fluoxetine or venlafaxine.
